Effective Date: 10/28/2026

Your Privacy Matters

Elevated Core Health is committed to protecting the privacy and confidentiality of your personal health information. This Notice of Privacy Practices explains how your health information may be used and disclosed, your rights regarding that information, and our responsibilities to protect it.

Please review this notice carefully. If you have questions about our privacy practices or your rights, please contact our office using the information provided at the end of this notice.

1. Our Responsibilities

We are required by applicable law to maintain the privacy and security of your protected health information (PHI). Protected health information includes information about your health, the health care services you receive, and payment for those services that can identify you.

We are required to provide you with this notice explaining our legal duties and privacy practices, follow the terms of the notice currently in effect, and notify affected individuals when a breach of unsecured protected health information requires notification under applicable law.

We will not use or disclose your protected health information except as described in this notice or as otherwise permitted or required by law. Certain uses and disclosures require your written authorization.

2. How We May Use and Disclose Your Health Information

We may use and disclose your protected health information for the following purposes, as permitted by applicable law.

Treatment

We may use or disclose your health information to provide, coordinate, or manage your health care and related services. This may include sharing relevant information with other health care professionals involved in your care when permitted by law.

Payment

We may use or disclose your health information to obtain payment for services provided to you. This may include billing, payment processing, eligibility verification, and other activities necessary to determine coverage or collect payment.

Health Care Operations

We may use or disclose your health information for health care operations, including quality assessment, practice administration, reviewing the effectiveness of services, staff training, and other activities permitted by law.

Appointment Reminders and Communications

We may contact you regarding appointments, scheduling, follow-up care, or other matters related to your treatment. We will make reasonable efforts to respect your communication preferences and protect the confidentiality of your information.

Business Associates

We may share your health information with service providers who perform functions on our behalf, such as billing, technology support, or administrative services, when permitted by law. These service providers may be required to safeguard your information under applicable agreements and legal requirements.

Legal Requirements

We may use or disclose your health information when required by federal, state, or other applicable law.

Public Health and Safety

We may disclose health information for certain public health activities, to report abuse or neglect when required or permitted by law, or to help prevent or lessen a serious and imminent threat to health or safety, as permitted by law.

Health Oversight and Legal Proceedings

We may disclose health information to authorized government agencies for legally authorized oversight activities. We may also disclose information in response to a court order or other lawful process when permitted or required by law.

Other Permitted Disclosures

We may use or disclose your health information for other purposes authorized or permitted by applicable law. When the law requires your written authorization, we will obtain it before making the disclosure.

3. Uses and Disclosures Requiring Your Authorization

Uses and disclosures of your health information that are not described in this notice or otherwise permitted by law will generally require your written authorization.

For example, certain uses and disclosures for marketing purposes, disclosures that constitute a sale of protected health information, and most uses and disclosures of psychotherapy notes require authorization when applicable law requires it.

You may revoke an authorization in writing at any time, except to the extent that we have already acted in reliance on that authorization or applicable law provides otherwise.

4. Your Rights Regarding Your Health Information

You have the following rights regarding your protected health information, subject to applicable legal requirements and exceptions.

Right to Inspect and Obtain Copies

You may request to inspect or obtain a copy of health information maintained in your designated record set. We may require your request to be submitted in writing.

We may charge a reasonable, cost-based fee when permitted by law. In certain circumstances, your request may be denied as permitted by law, and you may have the right to request a review of a denial.

Right to Request an Amendment

If you believe information in your health record is incorrect or incomplete, you may request an amendment in writing and explain the reason for your request.

We may deny your request in certain circumstances permitted by law. If we deny your request, we will provide an explanation and information about your available rights.

Right to Request Confidential Communications

You may request that we communicate with you about your health information in a particular way or at a specific location. For example, you may ask us to contact you at a particular telephone number or mailing address.

We will accommodate reasonable requests as required by law.

Right to Request Restrictions

You may request restrictions on how we use or disclose your health information for treatment, payment, or health care operations, or on disclosures to certain individuals involved in your care.

We are not generally required to agree to every requested restriction. However, we must agree to certain restrictions when required by law, including qualifying requests involving disclosures to a health plan for payment or health care operations when the relevant service has been paid for in full out of pocket and other legal conditions are satisfied.

Right to an Accounting of Disclosures

You may request an accounting of certain disclosures of your health information made during the period permitted by law.

The accounting does not include every disclosure. Certain disclosures, such as those for treatment, payment, and health care operations, may be excluded.

Right to a Copy of This Notice

You may request a paper copy of this Notice of Privacy Practices at any time, even if you previously agreed to receive it electronically.

Right to Choose Someone to Act for You

If you have granted someone medical power of attorney or if someone is your legal guardian or other authorized personal representative, that person may exercise certain privacy rights on your behalf as permitted by law.

We may verify the person’s authority before acting on a request.
